• The application deadline precedes the expiration of your license to allow the Board 30 days for processing the application for renewal of registration. The Board can only accept completed forms. Licensing is not renewed until all processing is complete.

An informal audit of nursing license renewals was conducted February 2000 and 22 out of 91 renewal applications were returned due to incompleteness of the application. Before mailing the renewal application, please make sure the following are complete:

• Credential category is checked
• If there is a name change, submit legal documents
• All questions must be answered
• If you have EVER been convicted of a misdemeanor or felony or had a disciplinary action taken against your license attach an accompanying letter of explanation and a certified copy of the court record (if conviction has been previously submitted, please state the type of conviction and date)
• List continuing nursing education hours and/or college hours
• Check or money order is enclosed
